The Office of the Registrar is an academic support unit of the Dominican College of Tarlac which manages operations in the areas of admission, registration, and records. It serves as the main front line service unit as regards curricular and academic matters.

Issuance of Diploma
Diploma may be issued only once. In case of lost/damaged original diploma, a duplicate copy may be requested by the graduate provided that a duly-notarized Affidavit of Loss must be submitted. Damaged original diploma must be returned to the Office of the Registrar. A duplicate diploma can be issued to a graduate who has no pending accountabilities with the school. In the event that a graduate may authorize a representative to do the said request. the representative is obliged to submit a letter of authorization and valid ID’s from both the owner and the representative to the in charge of request.
Issuance of Certification and Records
Certification and academic records can be issued to a pupil/student/graduate who has no pending accountabilities with the school. In the event that a pupil/student/graduate may authorize a representative to do the said request. the representative is obliged to submit a letter of authorization and valid ID’s from both the owner and the representative to the in charge of request. A duplicate certificate of registration may be requested by the pupil/student provided that a duly-notarized Affidavit of Loss must be submitted. Damaged original certificate of registration must be returned to the Office of the Registrar.
Certification, Authentication & Verification (CAV)
To obtain a Certification, Authentication & Verification (CAV) from the Department of Education (DepEd) and the Commission of Higher Education (CHED), the applicant will provide specific documents (i.e., Diploma, SF10 (F-137) / Transcript of Records) and submit the required documents to the designated DepEd/CHED regional office.
Procedures for Processing Expedite Request and Emergency Certificates Purposes
Submit proof of document (flight detail, job contract, job order, college admissions, etc.) In the event that requesting party may authorize a representative to do the said request, the representative is obliged to submit letter of authorization and valid ID’s from both the owner and the representative to the in charge of request.
The requesting party shall pay additional fee of 100% for expedited services.
For expedite services, 2 working days after payment.
